There is no one healthcare pathway for every British citizen living in Bulgaria. Your residence, employment, pensions and existing entitlement can change the arrangements. Confirm your position with the competent authorities before relocating.

Understand the kind of cover you need
Visiting Bulgaria is different from living there. A UK GHIC or EHIC is not a substitute for comprehensive travel insurance, and it does not mean a British citizen is automatically enrolled in Bulgarian state healthcare after moving.
For residence applications you may need specified insurance or proof of cover. Check the requirements of your actual residence route with the Bulgarian authority before buying a policy. If you are employed, self-employed, retired or receiving UK benefits, your basis for access to state care can differ.
Visitor cover, residence contributions and S1 registration are different.
A valid UK GHIC or qualifying EHIC can help eligible visitors obtain medically necessary state-provided treatment during a temporary stay. It does not cover private clinics or medical repatriation and is not a replacement for travel insurance. Confirm what your card actually covers before visiting.
If you settle in Bulgaria, healthcare rights may depend on your residence status, Bulgarian social-insurance contributions, an eligible registered S1 or qualifying private cover. An S1 is not automatically available merely because you are British, retired early or receiving a private pension. If you qualify, the certificate must be registered with Bulgaria’s National Health Insurance Fund (NHIF); check what cover applies until registration is completed.
Healthcare is not automatically free at the point of use even when someone has state-cover rights. Ask the provider and NHIF about patient contributions, specialist referrals, prescriptions and what happens if you use a private facility. Do not use historic lev-denominated treatment costs as current euro prices.

What exactly is an S1 certificate?
An S1 is an official healthcare entitlement certificate for some people who legally live in one European country but whose state healthcare costs are the responsibility of another. A qualifying UK-issued S1 lets you register for state healthcare in Bulgaria while the UK meets the relevant costs under international arrangements. It is not a visa, immigration permission, private insurance policy or a guarantee of free treatment.
Who might qualify?
Examples include eligible people receiving a UK State Pension or certain exportable UK benefits while lawfully resident in Bulgaria, some dependants, and qualifying posted or frontier workers. Receiving a private pension, retiring early, owning a Bulgarian house or simply being British does not establish S1 entitlement. Receiving a pension from more than one country can affect which state is responsible for your healthcare.
How to apply and register: the practical sequence
- Check your eligibility with the issuing authority. For UK State Pension or eligible exportable-benefit cases, use the NHSBSA S1 online application route ↗ or contact NHS Overseas Healthcare Services: +44 191 218 1999 (international; from the UK 0191 218 1999) or s1applications@nhsbsa.nhs.uk. Posted/frontier-worker cases normally start with HMRC, not the pensioner application route.
- Confirm when you can apply. Check NHSBSA's current online application requirements and intended start date before submitting; some S1 categories allow advance planning, but issuance and start dates depend on eligibility and receiving-country address evidence. If you have not started receiving the qualifying pension or benefit, check the NHS instructions rather than assuming advance entitlement.
- Supply the required personal and Bulgarian address details. An initial temporary Bulgarian address may be acceptable; notify the issuing body and the Bulgarian institution if your address changes. Confirm exactly what evidence the issuing body requests.
- Register the issued S1 promptly with your local Bulgarian National Health Insurance Fund (NHIF) office. An unregistered certificate is not the completed local healthcare registration. Obtain confirmation and ask how to enrol with an NHIF-contracted GP and access referrals, prescriptions and specialist services.
- Plan cover for any gap. Ask NHS Overseas Healthcare Services and NHIF what applies before the start date, during delays or while forms are being registered. Do not assume a visitor GHIC or a pending S1 will automatically cover a permanent move.
What does a registered S1 cover, and what doesn't it?
It generally gives eligible people access to Bulgarian state-provided healthcare on the same basis as an insured Bulgarian resident, including local co-payments, referral rules and limits. It does not promise UK-style free treatment everywhere, automatic private-hospital cover or repatriation. A registered eligible UK S1 holder may also have access to a UK-issued GHIC/EHIC for qualifying temporary travel and particular NHS visiting rights. Planned treatment abroad has separate rules, including the S2 route where applicable.
Families and changes in circumstances
Dependants must be assessed under the applicable rules, which can differ between the UK and Bulgaria. If someone was covered as a dependant and starts receiving their own UK State Pension, their previous dependant S1 can cease to apply; they should arrange and register the replacement S1 to prevent a gap. Tell NHSBSA about changes to address, employment, pension, dependant or residence circumstances. If another country's pension makes that country responsible, UK-funded entitlement may change.

If you may qualify for an S1
An S1 certificate lets some eligible people living in another country receive healthcare funded by the UK. It is relevant to some people receiving a UK State Pension and certain other qualifying circumstances. You cannot assume every pensioner or retired person is eligible.
Important pension and dependant edge case: Early retirement or a private pension alone does not establish entitlement to an S1. UK State Pension entitlement, certain exportable benefits, posted or frontier work, and dependant status can produce different outcomes. If your S1 cover comes through another person and you begin receiving your own UK State Pension, ask NHS Overseas Healthcare Services about a replacement S1 and register it with the Bulgarian NHIF to avoid a gap. Check the official UK healthcare guidance ↗.
First establish S1 eligibility with NHS Overseas Healthcare Services or the relevant UK body. If you are issued an S1, ask Bulgaria's National Health Insurance Fund (NHIF, НЗОК) how to register it. Keep confirmation of acceptance. Until the arrangements are valid, make sure you know which cover applies.
If you need Bulgarian health insurance
Bulgarian health insurance rules can depend on whether you work, your legal residence status and other factors. National Health Insurance Fund membership and paying privately for a medical appointment are not identical arrangements.
Check contributions, registration, coverage gaps and any required declarations with NHIF and the Bulgarian National Revenue Agency (NRA). Do not rely on an old quoted monthly contribution or assume an insurance purchase solves immigration requirements.
Medication and long-term conditions
Before leaving, discuss an ongoing treatment plan with your usual healthcare professional. Ask for a clear list of medication by active ingredient, dose and purpose where appropriate. Do not change or stop treatment without professional advice.
Check whether the medicine and an appropriate clinician are available locally, how prescriptions are issued in Bulgaria, and whether carrying the medicine is subject to restrictions. Keep supporting records securely and arrange continuity before your initial supply runs low.
Choosing a location for healthcare access
Research individual practices, pharmacies, hospitals and specialist services from your proposed address. A pin on a map does not prove a provider is accepting new patients, providing English-language care or offering 24-hour treatment.
For urgent medical emergencies Bulgaria uses 112. Check the official emergency procedures and make a personal contingency plan if you will be living far from a city or if winter access could be difficult.
What to ask before leaving the UK
If you are moving permanently, check with NHS or Overseas Healthcare Services how your entitlement changes and what UK-issued documents remain appropriate. Cover for an exploratory holiday is not the same as settled-resident cover.
Make a written comparison of public cover, any private insurance, likely co-payments, medication arrangements and emergency treatment rules. Keep renewal dates and contact details in a secure personal folder.
Prepare your healthcare plan.
- □ Confirm your residence status and applicable cover.
- □ Ask whether you qualify for an S1 and where to register it.
- □ Verify any insurance needed for residence permission.
- □ Check whether an appropriate clinician and pharmacy are available.
- □ Talk to your existing clinicians about medication continuity.
- □ Budget for contributions, co-payments and private appointments.
- □ Record emergency, specialist and insurer contact details.
